At JMED, we conduct routine health checks called PHA (phsycal health assessment) to support your healthly living.
To apply, please apply 2 to 3 weeks before requesting a check-up.
If you are taking PHA for the time at this clinic please register from here.
Already enrolled or had a check-up with us before.
Login and proceed to apply for a new checkup, to fill and change the questionnaire etc and to view the past results.

Payment Method
- Individual: You can pay by cash, credit card, check at reception.
- Corporate payment: We will send an invoice to your company after the checkup. In this case, you will need the name and signature of billing contact person at the time of application. Please specify the person in the application form.
- Insurance claims: Generally PHA are not covered by insurance, but some US insurance may cover. If you have such insurance please check the amount will be covered by it.
- Please note that the clinic will not be responsible for any amount owed by you or not paid by your insurance company or company after the medical checkup.
